Not all Doms fall in the nice stereotypes from the bad B movies. Some are different. Some like to gently arouse their lover (sub/slave) by gently kissing their nipples and lips and taking charge or her pleasure. This does not mean that more is not ever going to happen but that will be decided by them (? the Dom?). Discomfort can arouse but that is also an area that needs to be communicated by the sub to the DOM.
