All applications for service must include details of what you offer, and why I should want that. It should include a history of your service (or relationships if you have none) and what you expect from me.
Any skills you have - such as massage, languages etc you should explain how you acquired these skills and what level of training you have.
You should also bear in mind you will probably not be my exclusive sub and you may be required to serve me with others.
